The first five columns must have the header:

  1. Case
  2. EpisodeDate
  3. TestCode
  4. Result
  5. RefRange
  6. Units

Additional columns correspond to non-test attribute names.

The second and subsequent rows contain the values:

  1. The case name. Must be unique for each case. Can be formatted as text or numeric.
  2. The episode date and time. Must be unique for each unique episode. Must be formatted as an Excel date.
  3. The external name (i.e. lookup) of the test. Must be unique for each test. Must be formatted as text.
  4. The test result for the corresponding episode. Can be formatted as numeric or text.
  5. The normal range for the corresponding test result. Blank if there is no normal range. The formats are:
    • < x
    • ≤ x
    • > x
    • ≥ x
    • x – y
  6. The units for the corresponding test. Must be formatted as text. Blank if there are no units.

Example

The following spreadsheet corresponds to 2 cases. The first case has three episodes, with 2 tests on the earliest 2 episodes (16/05/2015 and 17/05/2015) and one test on the latest episode (12/06/2015).
