This type of question presents a list of options to the Intelligent Forms user. The user can select one or more of these options and the corresponding values will be inserted into the case as a comma-separated list.
If you specify that the question is “mandatory”, the Intelligent Forms user will be forced to select at least one option.
Each option consists of two parts, separated by a comma:
- the value that is to be inserted into the case if that option is selected, and
- the description that is shown for this option in the Intelligent Forms
You don’t have to supply a description, in which case the value is used also as the description.
For example, if you specify the following options:
Fort, Forteo (injections daily)
Ared, Aredia (injections 3 times a year)
Fosamax
then the descriptions shown to the Intelligent Forms user will be:
Forteo (injections daily)
Aredia (injections 3 times a year)
Fosamax
and the value inserted into the case if all three options are selected will be:
Fort, Ared, Fosamax
